Reaching Out to People with I/DD and Cancer
In cooperation with the American Cancer Society, Philadelphia Coordinated Health
Care, has implemented outreach efforts to people diagnosed with
intellectual/developmental disabilities (I/DD) and cancer.
The American Cancer Society offers many programs and support services to those
diagnosed with cancer. PCHC has made a commitment to help people diagnosed with
I/DD access those services and supports. We have done this by widely distributing
the
ACS Service Referral Form and the
Cancer Support Notification Form
in our region. By completing the ACS form, representatives from the ACS will contact
people with information about what services and supports are available. The PCHC
form allows us to follow-up on these requests as well as offer supports through
PCHC such as information and training.
We hope that people will take advantage of these supports if they are diagnosed
with cancer.
